| 1 | Pat Patterson The inaugural champion, recognized after a (kayfabe) tournament win in Rio de Janeiro unified the North and South American titles. | Sep 1, 1979Recognized as champion | 233 days |
WWE’s longest-tenured secondary title and historically a proving ground for future world champions. It was established on September 1, 1979 when WWF North American Heavyweight Champion Pat Patterson was recognized as having unified his title with an (entirely fictional) South American Heavyweight Championship. It is the third-oldest active title in WWE behind the WWE and United States Championships. Reign and champion totals vary slightly between sources depending on how vacancies, same-night changes and a brief 1999–2000 co-championship are counted; the lineage below follows WWE’s officially recognized history.
